: Ethical approval was obtained from the Medical and Health Research Ethics Committee (MHREC) Faculty of Medicine, Public Health and Nursing, Universitas Gadjah Mada, Indonesia, with approval number KE/FK/0757/EC/2018 and John Hopkins Bloomberg School of Public Health (JHSP) Institutional Review Board (IRB) (IRB number: IRB00008721). In addition, written informed consent was obtained from participants who agreed to have face to face interviews. Meanwhile, verbal consent was obtained from participants who wished to be interviewed via phone. Verbal consent was recorded before the interview was conducted. Face to face and phone interview have been stated in data collection methods in the study protocol. Phone interviews were conducted when the face-to-face interview was not possible, for example due to distance, and unmatched schedule. Therefore, phone interview and recorded verbal consent were conducted.
