: The study protocol received ethical clearance from the Institutional Review Board (IRB) of the Christian Medical College, Vellore, Tamil Nadu, India (<i>IRB no. 10691, dated 21.06.2017</i>). Data for the cross-sectional survey were collected by trained field workers using a pre-tested, interviewer-administered questionnaire after obtaining written informed consent from the parents of eligible children. Participants with lower literacy provided a thumbprint in place of their signature on the informed consent form in the presence of a literate witness during the cross-sectional surveys. Group verbal consent for participating in and audio-recording the FGDs was obtained by trained field workers prior to the commencement of each meeting. Only those participants who consented to participate in the study were enrolled. All study methods and procedures were conducted in accordance with the ethical principles of the Declaration of Helsinki.
