: The MCS secured ethical approval from the NHS MREC for each sweep (see and for further information and reference numbers) [, ]. All methods comply with the relevant national and institutional committees on human experimentation, including the ethical standards outlined in the 1975 Helsinki Declaration, as revised in 2013 [, ]. With cohort members under 18, written informed consent was required and obtained from parents/legal guardians. This study required no further ethical approval for secondary data analysis.
