We received ethical clearance from the University of Cape Town Commerce Ethics Committee on 17 April 2020 (REC 2020/04/017) and the Stellenbosch University Research Ethics Committee: Social, Behavioral and Education Research ethics committee on 25 May 2020. All survey participants were 18 and older. Verbal informed consent was obtained from all participants who enrolled for the telephonic interviews.
