: Informed consent was received from all study participants—written if the mother was literate and by thumb print if not. Consent received by both methods, both written and by thumb print, was approved by the ethics committee of the Institutional Review Board (IRB) of Development Media Internationals (DMI). Understanding that their participation was voluntary, participants could elect to terminate the survey at any time. The National Institute for Medical Research in Tanzania and relevant local government authorities authorized the research (TZ: NIMR/HQ/R.8a/Vol.IX/2344). Authorization for gathering data was approved by the IRB of DMI. All methods were performed in accordance with relevant guidelines and regulations.
