: This case study did not involve experimental protocols. Using their self-assessment tool, the UC Berkeley Committee for the Protections of Human Subjects determined that under Federal Regulations 21 CFR 50.3 and 45 CFR 46.102, these quality improvement activities were not deemed “human subjects research” and written, informed consent was not obtained. We confirm that all methods were carried out in accordance with relevant guidelines and regulations.
