: The design of this study has been reviewed by the Medical Ethical Review Committee of the VU University Medical Center Amsterdam who ordered that the Medical Research Involving Human Subjects Acts does not apply and granted an exemption from ethic approval (registration number: W16_276 # 16.324). The study will be performed in accordance with the 1964 Declaration of Helsinki and its later amendments or comparable ethical standards. Participation will be voluntary. Prior to the first interview, participants will be requested to provide written informed consent concerning both the interview and retrieval of information from their patient/client files. Participants will be in the opportunity to terminate their participation at any time and will receive a financial compensation of EURO 15 for each interview.
