: The EpiDoC study was performed according to the principles established by the Declaration of Helsinki (World Medical Association, 2013), revised in 2013 in Fortaleza. Ethical approval was obtained by the National Committee for Data Protection and by the Ethics Committee of the Faculty of Medical Sciences of the New University of Lisbon under the registration number 07–2011-CEFCM and 05–2012-CEFCM.
