The work described was carried out in accordance with the Declaration of Helsinki for experiments involving humans. The Ministry of Health and provincial and district government authorities arranged access to the selected households. Informed consent was obtained from all adults and children’s caretakers. After the trained surveyors explained the objectives and methodology of the study, consent was obtained from each participant.
