: Ethical approval was not required for this paper as we report on new analyses of previously published data aggregate data which was fully de-identified and anonymised. All participants who partook in research activities in the trials included in this paper provided written informed consent. Community members who received health interventions provided verbal consent for inclusion of their data in anonymised form for analyses of aggregate data. All four trials were conducted in accordance with the principles of the Declaration of Helsinki, with oversight by data and safety monitoring boards. Ethics approval for the BCPP trial was provided by the Botswana Health Research and Development Committee and the institutional review board of the Centers for Disease Control and Prevention trial. Ethics approval for the PopART trial was granted by ethics committees at the London School of Hygiene and Tropical Medicine, University of Zambia, and Stellenbosch University.
