: The institutional review board (IRB) at A.T. Still University’s Kirksville College of Osteopathic Medicine in Kirksville, Missouri, granted exempt status to this study under IRB Protocol 2019-092. The waiver for ethics approval covers all participating community health centers in the Preventing Heart Attacks and Strokes in Primary Care project that are members of health center controlled networks contracted for the project with the National Association of Community Health Centers. The waiver explicitly states, “This study will be conducted at community health centers in Florida, Missouri, Pennsylvania, Washington DC, New York, Illinois, California, Hawaii, Connecticut, New Jersey, and Oregon.” The reason for the waiver being granted is that the study is for quality improvement purposes and is a retrospective study that involves the review, analysis, and comparison of data from an existing database. Furthermore, participant data were de-identified and aggregated before being transferred to investigators.
