: The current research has been approved by the Norwegian Agency for Shared Services in Education and Research and by the Regional Committees for Medical and Health Research Ethics, South-East. The individual-level register data information were linked and deidentified by Statistics Norway, in accordance with Norwegian privacy legislation. The ethics committees have waived the requirement to obtain informed consent as the register data analysed in this study are in anonymised and deidentified format.
