: The study was performed in accordance with relevant guidelines and regulations (U.S. Department of Health and Human Services regulations for the protection of human subjects). In addition, this survey was approved by the Institutional Review Board (IRB) of the Afghanistan Ministry of Health (MoH). An informed verbal and written consent was obtained from all the participants/guardians/parents before the interview. We also sought permission from the DHS website and filled out a request to access and download the data. Therefore, further ethical approval to use the data is not necessary.
