: Ethics approval was received from the Queen’s University Health Sciences Research Ethics Board. A preamble to the survey explained its purpose, identified researchers, and described the background for the study. Informed consent to participate and for data to be published was implied by advancing from the study preamble to the survey, itself. The preamble also explained that any question could be left unanswered and that consent could be withdrawn at any time by not submitting the survey. All methods were performed in accordance with relevant guidelines and regulations and in accordance with the Declaration of Helsinki.
