: The study was approved by the research ethics committee of the Radboud University Nijmegen Medical Centre in July 2018 (decision number 2018–4238). After the letter of commitment was signed by the policy officer for each case, each potential participant was informed about the study individually. After agreeing to take part, each participant signed an informed consent form. All methods were carried out in accordance with relevant guidelines and regulations of studies involving human participants. All participants were coded with consecutive numbers, which replaced any personally identifying data. All audio- and videotapes and transcripts were stored on a save directory at the Radboud University medical centre.
: Consent for publication was obtained from all of the study’s participants. Participants were provided with information letters and were also verbally informed of the fact that any information they shared would be treated completely confidentialy and anonymously (e.g. without any personidentifiable information). This is in accordance with Dutch national guidelines.
