: Informed consent was obtained from all respondents to the survey. Statistics Denmark anonymizes and de-identifies the data before making it available to researchers. Use of the data for research purposes is allowed under Danish law for individuals affiliated with Danish research institutions without the need for ethical approval of individual studies (see for English versions of the relevant laws). The present study received ethical approval from Statistics Denmark’s Research Service Office under the auspices of data project no. 707676. No additional administrative permissions were required to access and use medical records or other material used in the study. All methods were carried out in accordance with relevant guidelines and regulations.
