: Ethical clearance for the study was obtained from the Ethical Review Committee (ERC) of University of Colombo (EC/2018/134) on 20th December 2018 and ERC North Western Province (NWP) on 26<sup>th</sup> December 2019. Administrative clearance was obtained from the Provincial Director of Health Services (PDHS), Regional Director of Health Services (RDHS), MOH and Divisional Secretariat (DS) of the Kurunegala district, Sri Lanka. Informed consent was obtained from all subjects prior to collect data and all methods were performed in accordance with the relevant guidelines and regulations.
