: Ethical approval for the IINDIAGO trial and component qualitative studies was granted by the Health Sciences Research Ethics Committee of the University of Cape Town (HREC reference: 946/2014, 829/2016), the Université de Montréal (CR CHUM: 2018–7091, 17.128-ID) and Witwatersrand (M170228). The relevant hospital authorities gave permission for the research and access to patient records. Written informed consent was obtained from all research participants and filed for safekeeping.
