: The participants were informed about the objectives of the project and informed consent was obtained from all participants. All subjects were above the age of 16 and if the subjects are illiterate, informed consent was obtained through a legal representative. All experimental protocols were approved by the institutional ethics committee of Vivekananda Memorial Hospital. All research involving human participants has been performed in accordance with the Declaration of Helsinki.
