The study was conducted from the US (online survey) and was not submitted to local ethics committees in the 5 European countries. The database NHWS was validated for exception determination by the Pearl Institutional Review Board in February 2018. Ethics approval not required for EU 5 for this type of database and study.All participants provided written informed consent as part of the survey process.
