: The study was reviewed by and received ethics clearance through a University of Waterloo Research Ethics Committee (ORE# 41477). Additional clearance for secondary analyses were received through a University of Ottawa Research Ethics Committee (H-06-20-5908).Parents/guardians with a potentially eligible child were provided information about the study and asked to provide informed consent for their child to participate. Only one child per household was invited to participate. The child was subsequently screened directly to confirm eligibility based on age and sex. Children aged 10 to 17 years were eligible to participate, with quotas for age and sex groups in the UK and US. Eligible children were provided with information about the study and were asked to provide assent before beginning the survey.
