: Approvals for the CoMix study questionnaires and protocols were initially obtained from the Ethics Committee of the London School of Hygiene and Tropical Medicine Reference number 21795 and through the local ethical committees for each individual country in the study. Informed consent was obtained via the internet from all subjects and their parents or legal guardians involved in the study. All the analyses were performed on pseudo-anonymised data. The full names of all the country-specific Institutional Review Boards (IRBs) that provided ethical approval are given in Supplementary Table S12 in Additional file. All methods were performed in accordance with the relevant guidelines and regulations.
