: This project was approved by the Lesotho Ministry of Health Ethics Committee (ID 107- 2020-Modify) and the University of Zambia Biomedical Research Ethics Committee UNZABREC, the Zambia National Health Research Authority and the London School of Hygiene and Tropical Medicine Ethics Committee. The Lesotho Ministry of Health Ethics committee allowed the study team to enrol participants with verbal consent. In Zambia, participants provided a signed informed consent form before being enrolled in the study while for participants 15–17 years both assent as well as parental consent were obtained. Research procedures were performed in accordance with the declaration of Helsinki.
