: This research was approved by the University of Lincoln Research Ethics Board, project reference 2292. The research involves the retrospective analysis of aggregate health data excluding identifying information, and excluding the records of patients who have requested to be excluded from health records collated by the East Midlands Ambulance Service. Consent was not obtained for individual patients because the research involves anonymised records only. Informed consent was waived because of the anonymised nature of the records by the University of Lincoln ethics committee. All methods were performed in accordance with the relevant guidelines and regulations.
