: Ethics approvals for this trial were obtained from the Deakin University Human Research Ethics Committee (2013–095), the Victorian Department of Education and Training (2013_002013), and the Catholic Archdiocese of Melbourne, Sandhurst, Ballarat, and Sale. An opt approach to recruitment was used. All children at participating schools and their parents/caregivers received information about the research and were enrolled in the study unless (a) the returned a signed opt-out form from their parents or (b) they chose to opt out prior to, or during, data collection. Students and their parents/guardians were considered to have provided informed consent unless parents/guardians returned a signed opt-out form to the school or students did not provide verbal assent at the time of measurement. Students were also able to participate in as much (e.g. all measurements) or as little (e.g. only survey) as they desired.
