: We used aggregate utilization data from the database of the Social Security Organization so informed consent was not required. The use of the database for this research purpose was approved by the Social Security Research Institute which granted permission to access and use the data (no: 2014012470). Also, the ethics committee of the Deputy of Research at Kermanshah University of Medical Sciences reviewed and approved the study protocol (IR.KUMS.REC.1402.3835).
