: This study followed the recomenations in the Declaration of Helsinki (World Medical Association, 2013) []. Ethical approval was obtained from the Linköping Regional Research Ethics Committee (Dnr 2017/211–31) in Sweden. All participants were informed about the study; that participation was voluntary; that they could withdraw their participation from the study at any time; and that all data would be handled confidentially. Written informed consent was obtained from all participants by the information letter attached to the questionnaire stating that by submitting the completed questionnaire, consent to participate in the study was given.
