: This study was approved by the UCT Human Research Ethics Committee (HREC 188/2016) and permission to conduct the study was received from the Western Cape Department of Health. The study adhered to the principles as set out in the Helsinki Declaration (2013) (13). All participants were given the participation information document (PID). Caregivers signed informed consent forms and signed consent for their children and adolescents to participate in the study, and children and adolescents signed the assent forms.
