: Ethics approval was granted by the Human Research Ethics Committee of the Federation University, Australia [C13–007], for secondary data analysis of de-identified data which had been collected by the primary data custodians. Verbal informed consent to the questionnaire was indicated by respondents’ agreement to participate in the telephone survey. In this government conducted national survey children were defined as aged less than 15 years, and the child questionnaire was completed by an adult parent or guardian. This research was conducted in accordance with the Declaration of Helsinki.
