: The current study was approved by the Danish Data Protection Agency (#1–16-02–683-18). The agency is an independent authority that overlooks research compliance with the rules on the protection of personal data. According to Danish Law (Act on Research Ethics Review of Health Research Projects), available at: (accessed on: 26 October 2022), questionnaire- and register-based studies do not require approval by ethics or scientific committees or written informed consent. However, the respondents in the current study gave informed consent by ticking a box in the questionnaire after being informed on the purpose of the project.
