: This study was based on anonymised individual-level register data and, therefore, the need for informed consent was deemed unnecessary according to national regulation. All methods were carried out in accordance with relevant guidelines and regulations. Ethical approval of this study was granted by the Swedish Ethical Review Authority (no. 2019–04376).
