The study was conducted in accordance with the ethical principles of Declaration of Helsinki. Written informed consent was obtained from all participants and they were assured of confidentiality of the information provided. For mothers younger than 18 years, written informed consent was obtained from their guardians. For the children and illiterate mothers, written informed consent was sought from their parents or guardians.
