Informed consent to participate was obtained from all of the participants in the study. Potential respondents were given information about the purpose of the survey, and asked “ Do you consent to take part in this survey?… Yes/No”. See supplementary file (survey). If they responded Yes, they were able to complete the questionnaire. The right of the participant to refuse to participate without giving reasons was respected. Participants were free to withdraw at any time and without giving reasons. Participants were notified of the publication plans and reminded that all their data were pseudonymised.
