: This study fulfilled the exemption criteria set by the MSF Ethics Review Board for a posteriori analysis of routinely-collected clinical data and thus did not require MSF ERB review. It was conducted with permission from the Research Committee, Operational Centre, Amsterdam, MSF. The study also fulfilled the exemption criteria of the Centre of Injury Prevention and Research ERB. All procedures were conducted in accordance with the Declaration of Helsinki and its amendments.
