: We obtained ethical approval from the Makerere University School of Medicine Research and Ethics Committee (SOMREC 2020 − 103), the Uganda National Council of Science and Technology (UNCST HS709ES), the National Health Science Research Committee in Malawi (19/11/2442), the Regional Ethics Committee of Norway (REK 30992), and the Liverpool School of Tropical Medicine (LSTM REC 19–105) institutional review boards. Permission was obtained from the hospitals where the study was conducted.
