: All methods were carried out in accordance with relevant guidelines and regulations (declaration of Helsinki). Use of the data was carried out under the same conditions and with the same purpose as collected. Participation in the study was voluntary and collected data were treated confidentially. The study was approved by the Danish Data Protection Agency prior to collecting the data (1-16-02-287-18). According to Danish Law -Act on Research Ethics Review of Health Research Projects, (The Act on Processing of Personal Data - Act No. 429 of 31 May 2000) questionnaire and register-based studies require neither approval by ethical or scientific committees.

: Informed consent was obtained from all the subjects involved in the study. The collected information was only used if the participant had given informed consent by ticking off a box with the following text at the beginning of the questionnaire: “I have been informed that I can receive a copy of my own answers and can have them deleted if I wish, and hereby agree to participate in the survey.” I chose to press “submit” after finishing the questionnaire.
