: Throughout the research process, basic principles of the research ethics were applied (The World Medical Association’s Declaration Helsinki 2004). The research protocol was approved by the Ethics Committee of the Finnish Institute for Health and Welfare (Statement THL/1390/6.02.01/2016). Statistics Finland gave the permission to use the register-based measures, and their rules and instructions were followed. All the statistical analyses on register data were conducted in a protected environment using a remote access system, and the results were screened by Statistics Finland before publishing. While presenting the results, the data were treated with strict anonymity to safeguard the identities of the respondents.
