: The study received ethical clearance from the Committee for Human Research and Ethics at the University of Energy and Natural Resources, Sunyani, Ghana (Reference: CHRE/CA/031/022). All procedures in this study were in accordance with the ethical standards of the Helsinki Declaration (1964, amended most recently in 2008) of the World Medical Association. All individuals aged 4 years and above were invited to participate in the study and recruited once they agreed and consented, or in the case of children (below 18 years), their parents consented. In addition, assent was sought for participants aged 10–17 years to affirm their willingness to participate in the study. A written informed consent form, either signed or thumb-printed and/or witnessed (when relevant), was obtained from all participants.
