: This study is a secondary analysis of previously collected and published household survey data. As such, it did not require independent ethical approval. However, each included survey obtained ethical clearance from respective national review boards, and informed consent was acquired from all participants. The specific consent procedures varied across surveys. Therefore, ethical approval and informed consent do not apply to this study. We obtained a copy of the ethical clearance for the combined datasets from the DHS Program.
