: Ethical approval was obtained from the Swedish Ethical Review Authority (EPM; Nos. 2020 − 01369, 2021 − 01124, 2024-02460-02). As this was a population register-based study using pseudonymized data, consent to participate was not applicable. However, opt-out messages were posted on the Lund University Population Research Platform (LUPOP) for participants to be able to express their decision to be excluded from the current study [ ]. The data were stored, managed and analysed at LUSEC, a high-security platform for handling sensitive data at Lund University.
