: Collection of individual and patient data in the Agincourt HDSS and health facilities received ethical clearance from the Human Research Ethics Committee (Medical) of the University of the Witwatersrand (protocols M960720, M110138 and M180585 for the Agincourt HDSS and protocol M151162 for health facility data ) and Mpumalanga Department of Health Provincial Ethics Committee. Written informed consent was sought from patients in the health facilities to collect and link their personal identifiers and clinical records with records held in the Agincourt HDSS population database.
