: The study was conducted in accordance with the ethical principles of the Declaration of Helsinki and the International Ethical Guidelines for Health-related Research Involving Humans (CIOMS, 2016) and complied with the Swedish Ethical Review Act (2003:460). Ethical approval was obtained from the Regional Ethical Review Board of Umeå, Sweden (2013–268 − 31ö and 2017/40132). The study used de-identified register data and questionnaire data collected as part of routine child health services.
