: This study was approved by the University of Witwatersrand Human Research Ethics (Medical) Committee (M2011106) and the Human Research Ethics Committee of the Ministry of Health in Oyo State, Nigeria (AD 13/479/3037). Participation in the survey was voluntary, and participants could withdraw from the study at any time.
