: The study was structured in accordance with the Declaration of Helsinki which deals with the recommendations guiding doctors in biomedical research involving human participants. The parent study received ethics approval from the three district and provincial ethics committees where the trial sites were located. This costing study was approved by the Human Research Ethics Committee (HREC) of the University of Witwatersrand (Ref 180705) and the University of Cape Town (Ref 452/2018), as well as, from the London School of Hygiene & Tropical Medicine (Ref 16107). We obtained written informed consent for study participation from all participants. Participants were not reimbursed.
