Ethics approval and consent to participate: Ethical approval for the study was obtained from the Institutional Review Board of Muhimbili University of Health and Allied Sciences (MUHAS) ( reference number: DA.282/298/01.C/2585). Written informed consent was obtained from all qualitative interview participants prior to participation. The study was conducted in accordance with the principles of the Declaration of Helsinki and the national ethical standards for research in Tanzania. Audio recordings and interview transcripts were stored on a password-protected hard drive. Audio recordings will be permanently deleted two years after completion of the study.
