Ethics approval and consent to participate: According to the Norwegian Health Research Act, this study did not constitute medical or health research as defined by the Act and was therefore exempt from review by the Regional Committees for Medical and Health Research Ethics (REK). Ethical principles for research were nevertheless followed, including ensuring informed consent, anonymity and treating participants with respect and minimizing the burden of participation as far as possible. Participation was voluntary, and informed consent was implied through completion of the survey.
