: The study was conducted in accordance with the Guideline for Ethical Aspects in Epidemiological Study (Ministry of Health, Labour and Welfare, Japan 2008). This study was approved by the Institutional Review Board of Tropical Medicine at Nagasaki University and the institutional review board of each participating hospital; Ebetsu City Hospital, Kameda Medical Centre, Chikamori Hospital, and Juzenkai Hospital. We used a standardized questionnaire form to provide patients and their guardians with necessary information. Written informed consent to participate in this study was obtained from participants or their guardians.
