: Premier Healthcare database was accessed through a commercial data licensing agreement where data used for the study was already de-identified prior to its release to study investigators, as set forth in the Data Use Agreement. Since patients cannot be contacted directly, obtaining informed consent was not applicable. The study database on health services research was fully compliant with the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule and federal guidance on Public Welfare and the Protection of Human Subjects (45 CFR 46 § 46.101). Institutional Review Board (IRB) approval was not required.
