: Both the qualitative and quantitative protocols were submitted to the Institutional Review Board Western-Copernicus Group (Puyallup, WA; references 1–1669153-1 and 1–1719988-1), and this study was determined exempt under 45 CFR § 46.104(d)(2). The study was conducted in accordance with the principles of the Declaration of Helsinki (Ethical Principles for Medical Research Involving Human Subjects). A third party vendor described risks to participation and obtained consent to participate in the qualitative interviews. For the on-line survey, consent was also obtained prior to participation (Additional file Survey Questionnaire).
