: The study was approved by the Unity Health Toronto Research Ethics Board (#19–231) and adhered to the Declaration of Helsinki. Each provider gave informed consent by signing an electronic consent form. All participating patients received a letter of information describing the study and providing an option to withdraw. However, patient-level written consent was waived by the Unity Health Toronto Research Ethics Board, as the intervention itself was a minimal risk quality-improvement intervention, and study outcomes were limited to physician-level behaviours.
